If you live within our practice area and wish to register please complete the forms required, available from reception or download them from the above “Online forms for Registration” tab & return them to the practice

If you need any help completing the registration documents please ask to speak to our Receptionist or Administration staff, they will be happy to help you.

You will be offered an New Patient Health Check appointment with our Health Care Assistant when you register.

Allocated GP

All patients are allocated a named GP, who has the overall responsibility for their care,  but you can still see any GP within the practice. If you wish to have a particular allocated GP or if you wish see a particular GP for your appointment, please inform the receptionist who will, subject to availability, try to meet your request.

Online Forms

You can download registration forms from the links below and then bring them into the practice to register with us.

Providing I.D. will help us to accurately match your details on the National Data Base (Spine) but you will not be refused registration if you cannot provide I.D.

Our administration staff will be happy to help you with your registration, just ring or call in to speak to them

New Patient Registration Form   (GMS1)
Use this form if you are living in the practice boundary area for longer than three months, or this is your permanent home address

AND

New Patient Questionnaire and Record Sharing Consent

When we receive your registration documents and on completion of the New Patient Questionnaire our administration team will process your request to register.

We also need to record your preferences for Data Sharing. Please complete this form and hand it in to the practice with your Registration Form - Thank you.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one and are staying within our practice boundary you can receive emergency treatment from the local GP practice. If your stay is going to be up to 3 months you can register as a "Temporary Resident" and if you are here for longer than 3 months you should complete a full registration form GMS1.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

All patients registered at Stanley Health Centre have been allocated a 'Named GP' who is responsible for that patients overall care at the practice.

If you wish to know who your Named allocated GP is or, if you have a preference as to which GP that is, the practice will try to accommodate this request. please contact us at the Health Centre.
